No. The form doesn't ask for it. We ask at your first visit, and only so we can plan around your dates.
How fast do you reply?
The front desk replies during practice hours, usually the same day. For anything urgent, call us.
Where do I park?
There is parking by the entrance of the building on Roswell Road, and an elevator to the second floor.
